Requirements for Replacement of Lost or Stolen Passport
Before filling out the required DS-11 and DS-64, please read the entire
instruction below.

Step 1 Gather and Prepare U.S. Passport Application Documents
Please note that only the online DS-11 application forms will be acceptable. Please click here for the online DS-11 Application Form - First Time (Complete but Do Not Sign) (After completing the form, you must click on the "CREATE FORM" button at the bottom of the page to generate a printable PDF version of the completed application). Please notice the following carefully:

Please click here for DSP-64 Application Form - Lost Passport Application Form For minors under age 14, both parents as well as the minor child should be present at the time of execution of the documents. If only one parent appears, the other parent must sign and notarize a consent form: Please click here for printing the Letter of Consent. Note: You may get passport photos at Kinkos, Costco, Walgreens or CVS. Click here for Authorization Letter in Acrobat Format One of the two signed Authorization Letter should be placed inside a sealed envelope by the Passport Clerk when executing the passport application
Step 2 Get Your Application Authorized All first time or new applicants need to have their applications authorized by local government officials called Acceptance Agents (AA). Bring all of the documents listed above to a Passport Acceptance Facility (click here to find the one closest to you) and notify the agent you are using an expediting courier service to "hand carry" your application. As stated, please present all above listed items to the Acceptance Agent for approval. You will then sign the passport application form in the presence of the Acceptance Agent who will witness your signature and select the following items from the above list to place in an envelope. The following documents should be placed inside the sealed envelope executed by the Passport Clerk:
Ask the clerk to execute the passport application documents and place them in a sealed envelope addressed to "U.S. Passport Agency." (You must not open this envelope again before shipping it.) If the agent feels hesitant about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications." *Make sure to print your name, date of birth, departure date on the outside of the sealed envelop.
