Requirements for New Passport Application

Before filling out the required DS-11, please read the entire instruction below.

You are eligible to apply for a new U.S. passport if:

  • You never had a passport and applying for a U.S. passport for the first time, or
  • Your expired US passport is not in your possession, or
  • Your passport is damaged or mutilated, or
  • Your previous US passport has expired and was issued more than 15 years ago, or
  • Your previous US passport was issued when you were under 16 years.

Step 1 Gather and Prepare U.S. Passport Application Documents

  1. Completing Application Form DS-11

    • Fully complete Application Form DS-11 - First Time Passport Application
    • Please note that only the online DS-11 application forms will be acceptable.

    (After completing the form, you must click on the "CREATE FORM" button at the bottom of the page to generate a printable PDF version of the completed application).

    Please notice the following carefully:
    1. Do not print the form on double sided pages.
    2. Do not sign this application form until you are instructed to do so during Authorization.
    3. After filling out and printing the form close the window to return to your passport instructions.

  2. Two Recent Passport Photographs

  3. Please note: acceptable passport photos are:
    • Photographs must be 2 x 2 inches in size, taken within the past 6 months.
    • The image size from the bottom of the chin to the top of the head should be between 1 inch and 1 3/8 inches.
    • The photographs must be in color.
    • They must be full face, front view with a plain white or off-white background.
    • Photographs should be taken without a hat or headgear that obscures the hair or hairline.
    • Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless required for medical reasons.
    • Home made Digital photographs are not acceptable
      • Note: You may get passport photos at Kinkos, Costco, Walgreens or CVS.

  4. Two Completed and Signed Authorization Letters

    One of the two signed Authorization Letter should be placed inside a sealed envelope by the Passport Clerk when executing the passport application
  6. Proof of U.S. Citizenship

    • It can be one of the following:
    • Original or Certified Birth Certificate - Issued by the County or State Health Department or Bureau of Vital Statistics. Hospital birth certificates are not accepted.
    • Consulate record of birth abroad
    • Naturalization Certificates also known as Certificate of Citizenship
    • Previous expired U.S. passport - issued over 15 years ago or issued when you were under the age of 16
    • Please note: when renewing passports for minors under age 15, both previous passport and original birth certificate should be submitted.

  7. Proof of Identification

    • State issued driver's license (at least 6 months old) bearing a photo, or military I.D. bearing a photo, If the above mentioned Driver's License isn't available, you will need to provide additional Proof of Identity.

  8. Proof of the Departure Date from the USA is Required

    • Copy of your plane ticket showing departure within 14 days
    • Computer generated flight itinerary from the travel agent or airline showing departure within 14 days
    • If your travel date is beyond 14 days from now, please call us first at 1-800-592-5918

  9. Complete Customer Passport Order Form

Step 2 Get Your Application Authorized

    All first time or new applicants need to have their applications authorized by local government officials called Acceptance Agents (AA). Bring all of the documents listed above to a passport acceptance facility and notify the agent you are using an expediting courier service to "hand carry" your application. As stated, please present all above listed items to the Acceptance Agent for approval. You will then sign the passport application form in the presence of the Acceptance Agent who will witness your signature and select the following items from the above list to place in an envelope.

    The following documents should be placed inside the sealed envelope executed by the Passport Clerk:

  • Your Proof of U.S. Citizenship
  • The Application Form DS-11
  • The Photographs (two photos required) The Appropriate Government fees ($135 for adults and $120 for minors under age 16, made payable to US Department of State. )
  • The Letter of Authorization
  • The Proof of Departure (Itinerary-travel within 14 days)

    Ask the clerk to execute the passport application documents and place them in a sealed envelope addressed to "U.S. Passport Agency." (You must not open this envelope again before shipping it.) If the agent feels hesitant about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications."

    *Make sure to print your name, date of birth, departure date on the outside of the sealed envelop.

Adults - Over age of 16 ($135 for US Dept. of State, $25 for execution clerk) $160.00
Minors - Under age of 16 ($120 for US Dept. of State, $25 for execution clerk) $145.00
