Crew Visa ("C" Visa)

Please collect, prepare and send the following documents:

1)   Passport: you must have and send us a valid passport with at least twenty months of validity left and one blank visa page (blank endorsement pages in your passport are not acceptable).

2)   Photo : one passport style color photo is required for each application

3)   Visa Application Form : please complete every item on the visa application form and sign it. Download the form here Visa Application Form ( in Acrobat Format | in Word Format ) . If you have trouble filling out the form, please click here   for a complete and step-by-step walk through.

4)   Documents: letter issued by your airline, cruise company, ocean liner, or transportation company, verifying your active status as a pilot, flight attendant, crew member, driver, and explaining your purpose of trip to China. The letter should be attentioned to Chinese Consulate in Los Angeles, printed out on company letterhead, and signed by a responsible corporate executive. 

-original employee ID. If a copy of an original employee ID is provided, it must be notarized first by a notary public.  

5)  Customer Order Form: complete this form in block letters Download here ( in Acrobat Format | in Word Format )

6)  Processing Fees: Personal check, company check, cashier's check, money order and personal credit card are accepted. Please make check or money order payable to TIMELY VISA, INC. 

*  If you submit multiple applications, please add $5 shipping fee for each additional passport.
*  Normally you are permitted to stay for up to 10 days for each entry to China .
